Before you begin
- Verify the intended patient, active encounter, existing symptom and clinical-write permission. Actual execution is Nurse on an isolated synthetic Inpatient encounter in a Polyclinic.
- Follow the facility’s approved clinical documentation and correction process. All pictured status changes and notes are fictional software exercises.
1. Reopen and identify the saved row
From encounter Overview, open the symptom edit control, or use Encounter Actions → Add Symptom. Allow the current records to refresh before editing. Identify the intended term and original onset. On an existing saved row, Onset Date is disabled; clinical status, severity, verification and note remain editable until an error correction is saved.

Actual Nurse in immutable original production UI, isolated synthetic Polyclinic active Inpatient encounter. Same fictional ICD-10-AM M54.5 problem-list item updated Active to Resolved, then corrected to Entered in Error; original onset/severity and two prior encounter diagnoses retained. No real patient or clinical decision. Other transitions, roles and facility combinations remain separate qualification work. Start: Existing Nurse symptom reloads with disabled original onset and editable clinical status, severity, verification and note.
- Existing onset is read only: Existing onset is read only
- Update clinical status: Update clinical status
- Review the saved note: Review the saved note
2. Choose the intended clinical status and update context
Open Status to review Active, Recurrence, Relapse, Inactive, Remission and Resolved. Clinical status is separate from severity and verification. The fictional example changes Active to Resolved, leaves Mild and Unconfirmed unchanged and replaces the note with an explicitly synthetic status-update explanation. This is still a pending draft.

3. Respond to a duplicate warning without creating another row
If Add another Symptom is used to select a code already present in a non-error row, the form warns Symptom already exists! and keeps the existing row. Review that row rather than adding a duplicate. In the example the pending Resolved update remains in the single row; the warning does not submit or save it.

- Duplicate symptom is blocked: Duplicate symptom is blocked
- Review the existing row rather than duplicating it: Review the existing row rather than duplicating it
4. Submit and verify the saved update
Review the complete pending row and select Submit. After returning to Overview, let the current data refresh and verify Mild, Resolved and Unconfirmed in Symptoms. Independent readback confirms that the same record UUID retained its 17 October onset, changed its status and note, and left the two existing encounter diagnoses unchanged.

5. Review the correction action carefully
Reopen the saved symptom and its three-dot menu. Verify the intended term before selecting Remove Symptom. For an existing record, this action immediately prepares Entered in Error in the form; this original UI has no separate confirmation dialog. It does not physically delete the saved record, and the prepared change still requires Submit.

6. Review and submit the entered-in-error correction
Review the pending Entered in Error verification and document the correction context in the note before submitting. The fictional example retains Resolved and Mild, adds a synthetic correction explanation and selects Submit. Entered in Error records an erroneous entry; it is different from Resolved clinical status.

7. Interpret the normal Overview and history filters
After the saved correction refreshes, the symptom is omitted from the normal Overview summary. Open See Clinical History → Past Symptoms. The example now shows No Symptoms / No symptoms have been recorded because that query excludes entered-in-error records. These filtered messages do not prove physical deletion or a negative clinical finding. Independent readback still finds the same corrected row and note.

8. Verify the retained read-only error record
Use Encounter Actions → Add Symptom to reopen the structured form and let current records refresh. The same symptom remains as a faded, disabled Entered in Error row with its original onset and saved correction context. Leave it unchanged. The demonstrated ordinary UI provides no reversal of that saved error row; use the facility’s approved correction and audit process for further action.

Troubleshooting
A previously saved value appears briefly after reopening
Wait for the current-record request to finish before editing or submitting. The observed form briefly presented cached values and then refreshed to the actual saved state.

The corrected symptom seems to have disappeared
