Current observations show the retained valid reading

Capture details

Exact committed release 8d99bb8a on an isolated nonproduction clone, using an existing Nurse and fictional Polyclinic inpatient case. The synthetic patient name retains an earlier release label. Ordinary Nurse current encounter list shows retained valid 74 only. Legacy72 still has actual Final status but its exact source is Entered in Error, so current clinical views exclude it. The pre-existing replacement note was authored on the earlier96ca source and is preserved verbatim; it does not describe the new filtering rule.

Legacy saved status and source error remain separate facts

Capture details

Exact committed release 8d99bb8a on an isolated nonproduction clone, using an existing Nurse and fictional Polyclinic inpatient case. The synthetic patient name retains an earlier release label. After normal horizontal table scrolling, the lower legacy 72 row displays actual saved Final plus separate Source form response entered in error. The upper valid 74 row displays Final without source-error label. This read-time rule preserves legacy records; it does not backfill their status. Identity/time/value remain in the paired before-scroll frame04.

Retained history distinguishes new Error, valid Final and legacy source-error Final

Capture details

Exact committed release 8d99bb8a on an isolated nonproduction clone, using an existing Nurse and fictional Polyclinic inpatient case. The synthetic patient name retains an earlier release label. After ordinary horizontal scrolling, the same three rows show: new 76 is Entered in Error with source-error context; valid 74 remains Final; legacy 72 remains actually saved Final with a separate source-error warning. Paired with the preceding measurement/time view, this does not imply any old record was backfilled.

  1. New 76 observation: Entered in Error: Reference targets the actual observation Error text. Its exact source response has the separate red source-error warning directly below; these are two distinct facts.
  2. Valid 74 observation: Final: Reference targets actual Final on the middle valid row, which has no source-error warning.
  3. Legacy 72 observation: actual saved Final: Reference targets actual Final on the bottom legacy row. The separate red source-error warning below explains its exclusion from current views; the old status was not backfilled.

Doctor distinguishes corrected and legacy source-error status facts

Capture details

Exact committed release 8d99bb8a on an isolated nonproduction clone, using an existing Doctor and fictional Polyclinic inpatient case. The synthetic patient name retains an earlier release label. The ordinary Doctor sees the same row statuses: new 76 observation Error with source Error, valid 74 Final, and legacy 72 actual Final with separate source-error warning. The paired left-hand history frame establishes each original measurement/time; no row is deleted or silently backfilled.

After correction, Pulse Recent Data refreshes to the one valid 74 measurement with its original 05:39 time and Nurse. New Error 76 and legacy source-error 72 are not current results; retained history is a distinct mode.

Exact committed release 8d99bb8a on an isolated nonproduction clone, using an existing Nurse and fictional Polyclinic inpatient case. The synthetic patient name retains an earlier release label.

  4. Explicit Full History is separate from current Graph and Recent Data: Explicit Full History is separate from current Graph and Recent Data. Retained history distinguishes new Error, valid Final and legacy source-error Final.

5. Verify the same retained records as the ordinary Doctor

With an existing authorized Doctor session, open the same patient and encounter. This step assumes ordinary account sign-in is already complete; general sign-in teaching is separate and is not illustrated here. Observations contains the one valid 74. On Pulse Full History, the same three original rows remain attributed to Nurse. Scroll to Status to confirm the same Error, Final and legacy Final-plus-source-error distinction. Doctor reading does not change the original author.

  2. Valid 74 observation: Final: Reference targets actual Final on the middle valid row, which has no source-error warning.

A correction is not deletion or a legacy backfill

The new source-linked correction changes only the new 76 observation’s status and appends its correction audit metadata. Legacy 72 retains its actual Final status and is excluded from current views because its source is Error. Valid 74 remains current. The interface presents the observation status and source validity as separate facts so retained old records are not presented as newly validated measurements.

Readability and scope remain explicit

This exact committed layout requires ordinary horizontal scrolling past long Notes to reach Status. The lesson preserves both genuine views; no screenshot is squeezed, cropped or edited to hide that limitation. A separate readability change is not part of these captures. This is one configured observation flow and two existing roles, not all vital types, diagnostic report history, external integrations or every facility’s authorization.
