Review the fields rather than inferring a completed event

Changing the start while Not Done is selected now preserves an empty end automatically. This observed behavior does not make every end control read-only, provide dedicated refusal-reason coding or establish a complete medication safety pathway. The saved status, occurrence and clinical context remain the information to inspect under facility policy.
